Well, whatever you do, don't pull on it. Has he been shedding lately? It almost looks like his whole underside was shedding and everything came loose except for what's around his vent. Give him a good long warm soak and see if that helps. If it really is something stuck in his butt, he is going to need to see a vet ASAP.

at first glance it looks like an undigested leaf of romaine 
but heck it should have some look of partial digestion..... How did the soak go?
