I work at an animal shelter as a vet tech. We came across an abandoned dragon that was so starved that it was flat as a pancake. It has MBD and I don't think a UVB light has ever touched him, only a heating lamp. I am very inexperienced when it comes to lizards as we almost never get them. Long story short, I have been nursing it back to health for a few weeks and grew attached to it and adopted it!! I need help sexing it!!

Best way to sex them is put them on a flat surface and lift the tail and gently pull it back to tighten the skin if before you get to the vent you see 2 ovals one on each side with a gap between that's a boy and his hemipenes..If you see a bump in the middle right above the vent that's a female.. You can also candle them in a dark room and in the same position place a flashlight on the top shining down thru tail you should be able to see the organs..
Looks like this critter needs some nice warm soaks and a soft soft toothbrush to help clean the pores and scales. Be careful not to hold them upside down very long as they have difficulty breathing in this position.

What the other two said! If you haven't already I recommend a long warm soak in a tub and a little scrub with a soft toothbrush, especially around those femoral pores on the legs. They look a little dirty. Not infected, just dirty. Also, a warm soak with help with whatever that is sticking out of the vent. It actually looks like it might be a sperm plug. My chameleon gets those and I just coax them out with a q-tip dipped in warm water.
